YAGレーザー照射による前部硝子体切開が奏効した悪性緑内障の1例

YAG-laser anterior vitreolysis was effective in a case with aphakic malignant glaucoma

　白内障手術後に生じた悪性緑内障に対して，片眼を前部硝子体切除，他眼をYAGレーザーで治療した1例を経験した．

　YAGレーザー治療眼では，術前，細隙灯検査で前部硝子体の厚さがうすいことが確認されたので，レーザー照射が施行された．後嚢切開後，前部硝子体が切開されたところ，後部硝子体腔から房水が前房へ流出し，短時間のうちに深い前房が形成された．

　このようなYAGレーザー治療は，必ずしも全例に奏効するとはいえないが，手術的操作の複雑さがなく，外来で簡単に行うことができるため，今後，無水晶体眼の悪性緑内障に対して，手術的治療を行う前に試みるべき有用な方法であると思われる．

A 87-year-old male underwent uneventful extracapsular cataract extraction for both eyes in 2 separate session. Malignant glaucoma developed in the right eye 4 months later. After argon laser iridotomy proved futile, anterior vitrectomy in-duced reformation of anteior chamber and normali-zation of intraocular pressure.

The left eye started to manifest shallow anterior chamber 5 months after cataract surgery. Dissec-tion of the residual posterior capsule and of the anterior vitreous by Nd-YAG laser resulted in prompt deepening of the anterior chamber.
