視野狭窄が進行した開放隅角緑内障に対するトラベクロトミー

Trabeculotomy for open angle glaucoma with advanced visual field constriction

　トラベクロトミーを施行した原発開放隅角緑内障（POAG）と水晶体偽落屑症候群を伴う開放隅角緑内障（PE）のうち，術前視野が湖崎分類Ⅲa以上に進行していた31眼（POAG 25眼，PE 6眼）を追跡調査した。平均術後経過観察期間は23.6か月で術前眼圧コントロール不良であった24眼のうち22眼92％が良好となった。視野が術後悪化した症例は3眼10％であった。術後高眼圧が続いた1眼を除き，視機能に影響を与える術後合併症はなかった。トラベクロトミーは，視野狭窄が進行した緑内障に対しても安心して施行できると思われた。

We performed trabeculotomy in 31 eyes with elevated intraocular pressure for primary open angle glaucoma or pseudoexfoliation. All the eyes manifested advanced visual field defects. The fol-low up after surgery averaged 2 years. Postsurgical level of intraocular pressure was kept below 20mmHg in 92% of the eyes. Further deterioration in visual field, as evaluated by Goldmann perimetry, occurred in 10%. There were no serious complica-tions of trabeculotomy that might affect the visual functions. Trabeculotomy seemed to be a safe and effective therapeutic modality for open angle glaucomas with elevated intraocular pressure and advanced visual field defects.
