We performed trabeculotomy in 31 eyes with elevated intraocular pressure for primary open angle glaucoma or pseudoexfoliation. All the eyes manifested advanced visual field defects. The fol-low up after surgery averaged 2 years. Postsurgical level of intraocular pressure was kept below 20mmHg in 92% of the eyes. Further deterioration in visual field, as evaluated by Goldmann perimetry, occurred in 10%. There were no serious complica-tions of trabeculotomy that might affect the visual functions. Trabeculotomy seemed to be a safe and effective therapeutic modality for open angle glaucomas with elevated intraocular pressure and advanced visual field defects.
