緑内障のある白内障眼への眼内レンズ挿入術—術1年後の視機能と眼圧の変化

Posterior chamber lens implantation in eyes with glaucoma. Postoperative results after one year

　緑内障のある白内障眼への眼内レンズ挿入術後1年間の視機能と眼圧の変化を検討した。対象は，当科で後房レンズ挿入術を受け，術後1年以上の経過観察が可能であった原発閉塞隅角緑内障（PACG）21例25眼と，原発開放隅角緑内障（POAG）29例38眼であった。視力はPACGの全例で改善し，POAGでは術後早期に眼圧上昇や黄斑部浮腫を認めた3眼以外は改善した。視野は術後1年間に湖崎分類で1段階進行したものがPOAGに3眼あった。眼圧調整が悪化したものは，PACGに3眼（12％），POAGに7眼（18％）あった。眼圧調整悪化眼では，眼内レンズが嚢内に固定されていないものが多く，眼圧調整悪化と後房レンズの嚢内以外の固定に，有意な相関があった。

Posterior chamber lenses were implanted in 25 eyes of 21 primary angle closure glaucoma (PACG) patients and 38 eyes of 29 primary open angle glaucoma (POAG) patients. Visual function and control of intraocular pressure (IOP) were evaluat-ed with a minimum follow-up of one year. Visualacuity improved in all eyes with PACG and 35 eyes (92%) with POAG. Three eyes (8%) with POAG had deterioration in visual field. Deterioration of I0P control was observed in 3 eyes (17%) of PACG and 7 eyes (18%) of POAG. A significant relation-ship was present between the deterioration of IOP control and the fixation of posterior chamber lens in the sulcus-sulcus or bag-sulcus.
