緑内障のある51眼への後房レンズ挿入術後の屈折値の変化と,術前予測値との差を調べ,SRK式およびSRKⅡ式の術後屈折の予測精度を検討した。SRK式を用いた結果では,術後6か月の時点での屈折値と術前の予測値とのずれが1ジオプター以下であったものは,PACGで21眼中13眼(62%),POAGで30眼中15眼(50%),対照で50眼中30眼(60%)あり,2ジオプター以下であったものは,PACGで20眼(95%),POAGで22眼(73%),対照で46眼(92%)であった。今回の症例にSRKⅡ式を適用した場合,術前予測値と術後屈折値のずれは,SRK式を用いた結果と比較して,PACGでは増加,POAGでは減少,対照ではほぼ不変であった。
We evaluated the state of refraction following posterior chamber lens implantation in 51 eyes with glaucoma. We employed the SRK formula in select-ing the power of intraocular lens to be inserted. The series comprised 21 eyes with primary angle closure glaucoma (PACG) and 30 with primary open angle glaucoma (POAG). The postoperative residual error was 1 diopter or less in 62% of PACG eyes, 50% of POAG eyes and 60% of 50 control nong-laucomatous eyes. The residual error was 2 diopter or less in 95% of PACG eyes, 73% of POAG eyes and 92% of control eyes. If we had employed SRK Ⅱ formula instead, the postoperative residual error would have been worse in PACG eyes, much better in POAG eyes and almost the same in control eyes.
