トラベクロトミーが有効であった虹彩振盪による色素性緑内障

Trabeculotomy was effective in an eye with pigmentary glaucoma secondary to iridodonesis

　亜脱臼水晶体による虹彩振盪が原因で隅角に色素沈着を起こし，そのために続発開放隅角緑内障を起こしたと考えられる5歳女児の症例を経験した。

　視神経萎縮，視野狭窄を認めたが，自覚症状はなく，たまたま発見された。

　薬物療法のみでは，眼圧コントロール不良で，トラベクロトミーのみを施行した。

　術後眼圧は，すみやかに降下し，交感神経β遮断薬点眼のみで，術後2年を経過して，眼圧コントロール良好である。

　トラベクロトミーは，線維柱帯を切開して，房水流出抵抗を低下させることにより，原発性，続発性を問わず，色素性緑内障に有効であると考えられる。

A 5-year-old girl presented with pigmentary glaucoma secondary to iridodonesis and subluxated lens in her right eye. The affected eye showed optic atrophy and visual field defect. As medications failed to control the intraocular pressure below 24mmHg, we performed trabeculotomy 4 months after her initial visit. In spite of persistent iridodonesis and subluxation of the lens, the intraocular pressure has been well controlled for 2 years after surgery.
