We evaluated the outcome of surgery for horizon-tal comitant strabismus in 123 adults aged from 15 to 77 years, average 31 years. The functional or cosmetic outcome was gratifying in 112 cases, 91%. We analyzed the surgical effect and rate of recur-rence by dividing the cases into 4 types: intermit-tent exotropia, constant exotropia, acquiredesotropia and congenital esotropia. The surgical effect was more in constant exotropia than in inter-mittent exotropia. It was independent of age. There was lesser incidence of recurrence in constant exotropia, esotropia and for all clinical types over 40 years of age. Persistent diplopia developed in 23%. It was more frequent in patients over 20 years of age. Overcorrection and anomalous retinal corre-spondence were major causes.
