To evaluate the efficacy of small-incision strabismus surgery, we performed recession-resection surgery on 23 patients with a muscle clamp through the modified cul-de-sac incision proposed by Guyton. Their ages ranged from 1 to 18 years. The far deviation ranged from 45 PD esotropia to 45 PD esotropia. This technique showed excellent results regarding cosmetic improvement and postoperative comfort. There was no difference in surgical accuracy between the small-incision and conventional limbal-incision approaches. The findings show that the small-incision strabismus surgery is safe and easy to perform. This technique is highly recomended as an alternative approach in strabismus surgery.
