We evaluated the feasibility to detect incipient rubeosis iridis with laser flare-cell meter. We per-formed fluorescein iris angiography in 76 eyes with proliferative diabetic retinopathy. Leakage of dye from the iris, considered as indicative of earliest stage of rubeosis iridis, was present in 36 out of the 74 eyes. Kubeosis iridis was overt in 9 eyes and occult in 27 eyes by slitlamp biomicroscopy.

We quantitated the aqueous flare with laser flare-cell meter. Flare reading was 20 photon counts or more in 24 eyes. Dye leakage from the iris was positive in 20 of the 24 eyes, 83%. Conversely, flarereading was 20 or more in 20 out of 36 eyes with dye leakage from the iris, 56%.

Flare reading was 15 photon counts or more in 40 eyes. Dye leakage from the iris was positive in 27 of the 40 eyes, 68%. Conversely, flare reading was 15 or more in 27 of 36 eyes with dye leakage from the iris, 75%.

The findings indicate that laser flare-cell meter is useful in detecting incipient rubeosis iridis. Flare reading over 20 is abnormal with false positive rate of 17% and false negative rate of 44%. Flare read-ing between 15 and 20 is borderline zone. Flare reading over 15 showed false positive rate of 32% and false negative rate of 25%.
