内頸動脈閉塞に伴う血管新生緑内障．虹彩ルベオーシスの発生機序と治療方針

Pathogenesis and therapeutic principles for rubeosis and rubeotic glaucoma secondary to carotid occlusion

　我々は内頸動脈閉塞症に伴う新生血管緑内障の3例を経験した。これらの症例は網膜所見に比して前眼部所見は高度であり，網膜冷凍凝固術，汎網膜光凝固術を施行したが無効であった。このことより内頸動脈閉塞症に伴う虹彩ルベオーシスの発生には網膜の虚血のみでなく，前眼部の虚血性変化も大きく影響していると考えられた。内頸動脈閉塞による虚血性眼症は，治療の困難な疾患である。したがって早期診断，早期治療が最も重要である。

We observed 3 cases of neovascular glaucoma secondary to ipsilateral occlusion of internal car-otid artery. The anterior uvea showed moreadvanced signs than the retina. Panretinal photocoagulation and retinal cryopexy was ineffec-tive for iris rubeosis. It appeared that ischemic changes in the anterioruvea as well as in the retina were contributory factors in rubeosis iridis in this entity.
