A 2-month-old male infant with bilateral Peters' anomaly presented with anterior staphyloma in the left eye. The eye had to be enucleated due to spontaneous perforation. Electron microscopy showed the corneal abnormalities to be more pronounced in the central than peripheral cornea. The central cornea showed absence of Descemet's membrane and endothelial cells. Immunohistochemical staining with anti-bodies to glycosaminoglycans demonstrated that keratan sulfate was present not in the central cornea but in the sclera anterior to the equator. These findings suggested disturbed migration of mesenchymal cells and differentiation in wide area of the anterior segment.
