小児白内障手術症例の検討

Review of cataract surgery in infants and children

　小児の白内障11例19眼に手術を行い，その長期経過を検索した。5歳以下の先天白内障7例12眼，6歳から15歳までのアトピー白内障3例6眼，9歳の外傷性白内障1眼であり，先天白内障5眼とその他の7眼には眼内レンズを挿入した。11眼（58％）で1.0以上の視力が得られた。角膜内皮細胞減少率は17.5％であり，両眼視機能もおおむね良好であった。眼球振盪2例，緑内障3眼，網膜剥離2眼，後嚢混濁17眼が主な合併症であった。術前に眼球振盪がない症例には眼内レンズ挿入術，眼鏡またはコンタクトレンズによる術後の屈折矯正，さらに必要があれば弱視訓練を行ったことが比較的多くの症例で良好な視力が得られたと判断される。

We evaluated the long-term outcome of cataract surgery in infants and children. The series com-prised congenital cataract in 12 eyes of 5 patients below 5 years of age, atopic cataract in 6 eyes of 3 patients aged from 6 to 15 years, and unilateral traumatic cataract in a 9-year-old child. Intraocular lens (IOL) was implanted in 5 eyes of congenital cataract and in all the other 7 eyes. Visual acuity of 1.0 or over was obtained in 11 eyes (58%). The corneal endothelial loss averaged 17.5%. The majority of cases attained good binocu-lar vision. Postoperative complications included nystagmus 2 cases, glaucoma 3 eyes, retinal detachment 2 eyes and opacity of posterior capsule 17 eyes. The relatively fair visual outcome in the majority of cases was due to IOL implantation in eyes without nystagmus, correction of refraction with eyeglasses or contact lens, and orthoptic training whenever necessary.
