アンホテリシンB硝子体内注入を併用した内因性真菌性眼内炎の3例

Three cases of fungal endophthalmitis treated by intravitreal amphotericin B

　腹部外科手術後に高カロリー輸液療法を受けている3症例の両眼に真菌性眼内炎が発症した。全例にフルコナゾール全身投与を行ったが4眼で眼内炎が改善しなかった。フルコナゾール全身投与をさらに続行し，硝子体混濁が強い3眼に硝子体手術とアンホテリシンBの硝子体内注入，1眼にアンホテリシンBの硝子体内注入を行い，眼内病変が消退した。フルコナゾール全身投与に抵抗する内因性真菌性眼内炎に対して，アンホテリシンBの硝子体内注入の併用が奏効した症例群である。

Endogenous fungal endophthalmitis developed in both eyes of 3 patients. These patients had been receiv-ing intravenous hyperalimentation after abdominal surgery. The endophthalmitis failed to respond to systemic flucona-zole in 4 eyes. Three eyes were treated, additionally, by vitrectomy and intravitreal injection of amphotericin B. One eye received intravitreal injection of amphotericin B. Endophthalmic lesions subsided in these 4 eyes. The findings show that additional intravitreal amphotericin B may be effective for fungal endophthalmitis resistant to systemic flu-conazole alone.
