We reviewed 18 eyes of 16 patients who underwent deep lamellar keratoplasty. The causative lesion comprised lattice corneal degeneration 7 eyes, corneal leukoma 6 eyes, and others. Fresh donor cornea was used throughtout. The postoperative follow-up ranged from 6 to 82 months, average 22 months. The graft remained transparent in 14 eyes (78%). The remaining 4 eyes received a second surgery. The visual acuity improved in all the eyes. Visual acuity of 0.3 or over was obtained in 14 eyes (78%). The corneal astigmatism averaged 2.4±1.3 diopters at 6 months after surgery. The findings show that deep lamellar keratoplasty is effective for corneal lesions with relatively intact corneal endothelium because of few severe complications, lesser corneal deformity, and lesser incidence of graft opacification.
