糖尿病網膜症に対する硝子体手術後の眼圧上昇

Elevation of intraocular pressure following vitrectomy for proliferative diabetic retinopathy

　初回硝子体手術を行った血管新生緑内障が合併しない増殖糖尿病網膜症129例154眼での術後眼圧上昇を遡及的に検討した。51眼（33.1％）で術後眼圧が22mmHg以上に上昇した。眼圧上昇は，37眼（73％）で術後7日以内，4眼（8％）で1か月以降に起こった。眼圧上昇眼のうち26眼（51％）では無処置で眼圧が正常化した。30眼（59％）では7日以内に眼圧が正常化し，9眼では緑内障に移行した。眼圧上昇の危険因子は，低年齢，術前の網膜剥離の存在，術後の前房出血であった。

We reviewed 154 eyes in 129 patients regarding elevation of intraocular pressure (IOP) followingprimary vitrectomy for proliferative diabetic retinopathy. Eyes were excluded manifesting neovascular glaucomaprior to surgery. IOP rise over 22mmHg developed in 51 eyes postoperatively, in 37 eyes (73%) within 7 daysand in 4 eyes (8%) after 1 month of surgery. The IOP normalized spontaneously in 26 eyes (51%). In 30 eyes (59%) , it normalized within 7 days of surgery. Glaucoma persisted in 9 eyes. Risk factors for postoperative IOPrise were young age, preoperative retinal detachment and postoperative hyphema.
