We reviewed 15 eyes of 15 cases who underwent vitreous surgery for bacterial endophthalmitis following intraocular lens (IOL) implantation. Visual acuity at the time of surgery was hand motion or less in 12 eyes (80%). Endophthalmitis became manifest 1 to 19 days after IOL implantation. It developed within 4 days in 10 eyes (67%). The final visual acuity ranged from 0 to 1.0, including 10 eyes (67%) showing 0.5 or better. Six eyes with absent posterior lens capsule tended to result in poor visual outcome. The causative organisms could be identified in 8 eyes (53%). Final vision was lost in one eye each due to Enterococcus faecalis and α-Streptococcus. Retinal detachment developed in 4 eyes after vitreous surgery. Reattachment was obtained by additional surgery in 3 eyes. Surgery was refused in the other 1 eye. The findings show that vitreous surgery is effective for bacterial endophthalmitis following IOL implantation.
