A 69-year-old female presented with decreased vision in her right eye since one year before. Hercorrected visual acuity was 0.02 right and 0.7 left. The right eye showed tilted disc syndrome and stage 4 macularhole after classification by Gass. The left eye showed normal findings except incipient cataract. We treated herright eye by phacoemulsification-aspiration, intraocular lens implantation, vitrectomy, scraping of the retinalpigment epithelium within the macular hole and vitreal injection of SF6gas. No membranous structure wasidentified anterior to the macular area. Macular hole closed following surgery. At 8 months after surgery, herright visual acuity was 0.1. The hole area was filled by pigmented mass surrounded by subretinal fibrotic neovascularmembrane. Surgically removed neovascular tissue showed accumulated pigment cells posterior to the foveasurrounded by fibrotic neovascular membrane. It appeared that choroidal neovascularization in this patient wasdue to damaged Bruch's membrane secondary to tilted disc syndrome and to intraoperative scraping of retinalpigment epithelium.
