術前視力不良例の糖尿病黄斑浮腫に対する硝子体手術成績

Outcome of vitreous surgery for diabetic macular edema with poor visual acuity

　術前矯正視力が0.1未満の糖尿病黄斑浮腫に対ずる硝子体手術の成績を検討した。対象は後部硝子体剥離の認められない糖尿病黄斑浮腫19例26眼で，全例で術中に人工的後部硝子体剥離を完成させた。最終受診時に黄斑浮腫の消失・軽減を認めたのは26眼中20眼（77％）で，そのなかで視力の改善が得られたものは9眼にとどまった。術前視力不良例でも硝子体手術で視力改善することはあるものの，良好な視力を得られる可能性は低いと思われた。

We reviewed 26 eyes of diabetic macular edema treated by vitreous surgery. All the eyes had visualacuity of less than 0.1. No posterior vitreous detachment was present. All the eyes underwent artificial posteriorvitreous detachment. After follow-up for 6 to 55 months, average 27 months, the macular edema decreased orresolved in 20 eyes (77%). The visual acuity improved in 9 eyes. The findings show that vitreous surgery maybe effective for diabetic macular edema but may induce improved visual acuity in a small number of eyes whenthe preoperative visual acuity is poor.
