インドシアニングリーンを用いた内境界膜剝離後に鼻側視野欠損を生じた3例

Three cases of nasal visual field defect following indocyanine green-assisted peeling of internal limiting membrane

　特発性黄斑円孔3例3眼にインドシアニングリーン（indocyanine green：ICG）を使った内境界膜剝離を行い，鼻側視野欠損が生じた。2例では水晶体超音波乳化吸引術と眼内レンズ挿入を同時に行った。これら3眼すべてに，鼻側視野欠損の原因になりうる病変は，術中・術後ともになかった。当院では，今までのICGを使わない同様の手術では，このような鼻側視野欠損は起こっていない。視野欠損の原因として，ICGの網膜に対する毒性の関与と，今回の3例すべてが左眼発症であり，右利きの術者が左手で光源を操作していたために耳側がより強く露光された可能性がある。

　Nasal visual field defect in 3 eyes after vitreous surgery for idiopathic macular hole. All the eyes received indocyanine green-assisted peeling of internal limiting membrane. Two eyes also received phacoemulsification-aspiration and intraocular lens implantation. There were no circumstances during or after surgery that would result in nasal visual field defect. No such instance of visual field defect has been observed by us following similar surgery without adjunct use of indocyanine green（ICG）. There is a possibility that the visual field defect was due to still unidentified toxicity of ICG.
