　73歳の女性が1週前からの右眼充血で受診した。右眼には虹彩炎と雪玉状の硝子体混濁があり，両眼の隅角に周辺虹彩前癒着があり，肉芽腫性ぶどう膜炎の所見を呈していた。リゾチームとγグロブリンの血清値が上昇し，サルコイドーシスが疑われたが，PCR法で前房水にEpstein-Barr （EB）ウイルスDNAが検出され，診断が確定した。ステロイド薬の点眼と結膜下注射でやや軽快した。アシクロビル全身投与は無効であった。本症例は，治療に抵抗する慢性肉芽腫性ぶどう膜炎にEBウイルスが関与する可能性があることを示している。

A 73-year-old female presented with hyperemia in her right eye since one week before. Her right eye showedsigns of iritis and snowball vitreous opacity. Both eyes showed peripheral anterior synechia simulating granulomatousuveitis. Elevated serum levels of lisozyme and gamma-globulin were suggestive of sarcoidosis. Final diagnosis wasobtained after Epstein-Barr virus DNA was detected in the aqueous processed by polymerase chain reaction. Topicalcorticosteroid induced slight improvement in ocular findings. Systemic acyclovir was futile. This case illustrates thatEpstein-Barr virus infection may be involved in chronic granulomatous uveitis resistant to conventional therapeuticmodalities.
