緑内障・白内障・後房眼内レンズ同時手術の成績

Simultaneous surgery for glaucoma and cataract with posterior chamber lens implantation

　Trabeculectomyまたはtrabeculoto-my，白内障嚢外摘出術，後房眼内レンズ移植の三手術同時手術，いわゆるtriple procedureを24例33眼に施行し，術後3か月から3年（平均14.7か月）の経過を観察した。原発開放隅角緑内障と水晶体嚢偽落屑症候群に対してはtrabeculotomyを選択し，原発閉塞隅角緑内障・続発緑内障・虹彩前癒着の強い症例・濾過手術既施行例に対してはtrabeculectomyを選択した。術後，視力は73％で2段階以上改善し，眼圧は88％が20mmHg以下にコントロールされ，投薬は70％で減少し，視野は91％で改善または進行しなかった。

We treated 33 eyes with cataract and glaucoma by simultaneous triple procedure of trabeculectomy or trabeculotomy, extracapsular cataract extrac-tion and posterior chamber lens implantation. Trabeculectomy was mainly performed in eyes with primary open angle glaucoma and pseudoex-foliation syndrome. Trabeculectomy was indicated for primary angle closure glaucoma, secondary glaucoma and for eyes with previously operated glaucoma. The patients were followed up for 3 months to 3 years, median 14.7 months. After sur-gery, the visual acuity improved in 73%, intraocu-lar pressure was kept at or under 20 mmHg in 88%, the number of medication was reduced in 70%, and visual field loss did not progress in 91% of the eyes.
