中心性輪紋状脈絡膜ジストロフィに脈絡膜陥凹をきたした症例のOCT所見

A case of central areolar choroidal dystrophy with focal choroidal excavation by optical coherence tomography

要約　目的：脈絡膜陥凹のある中心性輪紋状脈絡膜ジストロフィの症例の報告。症例：51歳男性が，4年前からの視野欠損で受診した。10年前に健康診断で，両眼の黄斑部異常を指摘された。夜盲はない。所見：両眼に約－6Dの近視があり，矯正視力は左右眼とも1.2であった。両眼の黄斑部に境界鮮明な類円形の萎縮があった。フルオレセイン蛍光造影（FA）で，萎縮巣に一致した低蛍光があり，光干渉断層計（OCT）で，網膜外層と脈絡膜の菲薄化があり，萎縮がより進行した部位に脈絡膜陥凹があった。2年後に視力がやや低下した。結論：中心性輪紋状脈絡膜ジストロフィのOCTによる所見として，脈絡膜の菲薄化と脈絡膜陥凹があった。

Abstract.　Purpose：To report a case of central areolar choroidal dystrophy who showed focal choroidal excavation by optical coherence tomography（OCT）. Case：A 51-year-old male presented with visual field defect in both eyes since 4 years before. He had been diagnosed with abnormal finding in the macula in both eyes 10 years before. He was free of night blindness. Findings：Visual acuity was 1.2 in either eye when corrected by －6 diopters. Both eyes showed oval-shaped and sharply demarcated atrophy in the macula. Fluorescein angiography（FA）showed hypofluorescence in the atrophic area. OCT showed attenuated outer retinal layer and choroid. Choroidal excavation was present at the site with more advanced atrophy. The visual acuity decreased slightly 2 years later. Conclusion：In both eyes with central areolar choroidal dystrophy, OCT showed choroidal attenuation with choroidal excavation.
