We observed spontaneous regression of polypoidal choroidal vasculopathy in two cases, a 73-year-old female and 59-year-old male. Both were unilaterally affected. The corrected visual acuity was 0.15 and 0.6 each. The polypoidal lesions disappeared after 21 and 9 months each following regression of exudative macular lesions. The visual acuity improved to 0.5 and 1.0 each. Indocyanine green angiography showed hypo-fluorescent area followed by spontaneous disappearance of polypoidal lesions, although alnormal vasculor net-work did not disappear even after the disappearance of the polypoidal lesions. Optical coherence tomography (OCT) showed, during the course of observation, flattening of elevated retinal pigment epithelium and in-creased reflectivity in the polypoidal lesions followed by diappearance of retinal detachment. These findings seemed to show that regression of polypoidal lesions was induced by thrombotic cicatriazation of aneurysmal lesions.
