涙腺多形腺腫症例の臨床的検討

Clinical features of pleomorphic adenoma of the lacrimal gland

　過去14年間の原発性眼窩腫瘍35自検例のうち，9例が涙腺多形腺腫であり，これを解析した。年齢は19歳から63歳，平均49.O±14.0歳であった。性差，左右差はなかった。発症から当科受診までの期間は5か月から10年，平均4.4±3.7年であった。初発症状には眼球突出と腫瘍触知が多かった。全例に腫瘍摘出術を行った。8例で再発はなく，他施設での腫瘍摘出後に再発した1例が当科で手術を受け，10年後に再発したが，悪性化はなかった。涙腺多形腺腫では，再発と悪性化を防ぐために，初回手術で被膜ごと完全に摘出することが重要である。

We reviewed 9 cases of pleomorphic adenoma of the lacrimal gland seen during the past 14 years. A total of 35 cases of primary orbital tumor were seen during the same period. The age ranged from 19 to 63 years, average 49.0±14.0 years. The 9 cases comprised 5 males and 4 females. All the cases were unilaterally involved. The tumor was located in the right side in 5 cases and in the left in 4 cases. The interval between the onset and visiting us ranged from 5 months to 10 years, average 4.4±3.7 years. Proptosis and palpable mass were the most frequent symptoms. All the 9 cases underwent extirpation of the tumor. There has been no recurrence in 8 cases. The other 1 case recurred 10 years after surgery with no evidence of malignancy. We advocate complete removal of the tumor at the initial surgery to prevent recurrence or malignancy.
