再発をくり返し，悪性化した涙腺多形腺腫の1例

A Case of Recurrent Adenocarcinoma in Pleomorphic Adenoma of Right Lacrimal Giand

緒言

　多形腺腫—いわゆる混合腫瘍—の多くは唾液腺に発生し，涙腺の多形腺腫の発生頻度は全多形腺腫の0.5〜1.0％にすぎない^1）。しかし，涙腺多形腺腫は臨床的に遭遇する機会の比較的多い腫瘍の一つであり，全涙腺腫瘍の40〜90％の発生頻度が報告されている^2〜3）。多形腺腫は一般に腫瘍を完全に摘出すれば根治しうるという意見が多いが，さらに長期間にわたり術後経過を観察すると，病理組織学的に良性と診断された場合にも，再発ないしは悪性化の転帰を示す症例も決して少なくはないことも報告されている^4）。

　著者らは第1回の手術後34年の経過中に，再発をくり返し，その都度手術を受け，最終的には悪性化し，局所への浸潤，および肺への遠隔転移を示して死亡した右涙腺多形腺腫の1例を経験したのでここに報告し，併せて再発例の手術方法に関して考察を加えた。

A 49-year-old man died from generalized me-tastasis of lacrimal gland tumor. This case is worthy of report because of extraordinary long history of evolution of the tumor.

The first sign of the tumor was noticed at age 15 by proptosis of the right eye. At age 17 the tumor was thought to be of lacbimal gland origin and was surgically removed. The histo-logical diagnosis was pleomorphic adenoma. The tumor repeatedly recurred and surgical extirpa-tion was performed at ages 25 and 38 with the same histological findings. When it recurred at age 48, metastasis to lung and local extension to bone and skin were noted.
