多発性の涙腺多形腺腫の1例

A case of multifocal pleomorphic adenoma of the lacrimal gland

　68歳女性が6年前からの右上眼瞼と側頭部の腫脹で受診した。27年前に右眼窩腫瘍で腫瘍摘出術が行われ，病理診断は多形腺腫であった。右上眼瞼耳側と右側頭部に表面平滑で弾性硬，可動性のある腫瘍を触れた。腫瘍摘出術を行い，両腫瘍とも良性多形腺腫と診断された。涙腺多形腺腫は比較的頻度の高い涙腺原発の腫瘍であるが，これが多発することは稀である。耳下腺の多形腺腫の再発例では組織学的に6割以上に多発性腫瘤があるとされているが，涙腺ではほとんど報告例がない。過去の腫瘍摘出が不完全であったための再発と考えられる。

A 68-year-old woman presented with swelling in the right upper eyelid and right temple since 6 years before. She had received excision of right orbital tumor 27 years before. It had been diagnosed with pleomorphic adenoma. Palpation showed flat, elastic hard and mobile tumors in the right upper eyelid and right temple. Surgical excision showed benign pleomorphic adenoma in both lesions. Multifocal occurrence of pleomorphic adenoma of the lacrimal gland is thought to be very rare. Incomplete extirpation of primary pleo-morphic adenoma in the past would have resulted in multifocal recurrence in this patient.
