インターフェロンβ投与により発症した腎性網膜症の1例

A case of renal retinopathy following systemic treatment with interferon β

　33歳女性が4年前にC型肝炎と診断され，インターフェロンβの投与開始直後に受診した。矯正視力は正常で，近視以外に眼科的に異常はなかった。ネフローゼ症候群が発症し，投与は7週目で中止されたが，その5日後に矯正視力が右0.6、左0.1に低下した。両眼の乳頭周囲に軟性白斑が多発し，網膜の表層性出血，黄斑浮腫があった。さらに5日後に視力が右0.15，左0.06に低下した。星芒状白斑が出現し，腎性網膜症と診断した。5か月後に視力と眼底所見がほとんど正常化した。本症例は，インターフェロンβの投与により腎性網膜症が起こり得ることを示している。

A 33-year-old female was seen by us immediately after initiation of systemic treatment with interferon β for hepatitis C of 4 years' duration. She was ophthalmologically intact except for myopia. Interferon had to be discontinued for nephrotic syndrome 7 weeks later. When seen 5 days later, her corrected visual acuity was 0.6 right and 0.1 left. Funduscopy showed multiple soft exudates around the disc, superficial retinal hemorrhages and macular edema. The visual acuity deteriorated to 0.15 right and 0.06 left another 5 days later. Macular star figure had developed. She was diagnosed as renal retinopathy. The visual acuity and fundus findings almost normalized 5 months later. This case illustrates that renal retinopathy may develop following systemic treatment with interferon β.
