水晶体嚢外摘出後に生じた悪性緑内障の両眼例

A bilateral case of malignant glaucoma following extracapsular cataract surgery

　水晶体嚢外摘出術後に両眼に悪性緑内障を発症した80歳女性の1例を経験した。両眼とも閉塞隅角緑内障の既往を有し，眼軸長は短かった。無水晶体眼である右眼はNd-YAGレーザーによる前硝子体膜切開術にて深い前房を得た。眼内レンズ挿入眼である左眼は前部硝子体切除術および経毛様体扁平部濾過術の同時手術で良好な成績を得た。

A 81-year-old female presented with elevated intraocular pressure (IOP) one month after extracapsular cataract extraction and peripheral iridectomy for acute closed angle glaucoma in her right eye. Capsulotomy and anterior vitreous membranotomy with Nd: YAG laser resulted in reformation of anterior chamber and normalization of IOP. The axial length measured 21.20 mm in the right eye and 21.27 mm in the left. The left eye was treated by extracapsular cataract extraction and intraocular lens implanta-tion for intumescent cataract and secondary glaucoma. Malignant glaucoma developed 2 weeks later and was successfully treated by anterior vitrectomy and filtration through the pars plana.
