We diagnosed two females, aged 40 and 77 years, as unilateral trochlear palsy following thorough clinical examinations including prism cover test in 9 directions, quantitation of rotation using a red glass and tangent screen of Harms, and three-step test of Parks. Surgery to one of the vertical rectus muscles induced orthotropia and disappearance of diplopia. Lid ptosis and vertical deviation recurred later in both cases. Further systemic examinations including tensilon test led to the diagnosis of late-onset myasthenia gravis. These cases illustrate that myasthenia gravis may be an underlying cause of apparent palsy involving a single or multiple extraocular muscles.
