A 22-year-old female presented with exotropia, bilateral visual field defect and unilaterally enlarged optic disc.Magnetic resonance imaging showed periventricular leukomalacia.It appeared that intracranial hemor-rhage during the perinatal period caused periventricular leukomalacia followed by optic nerve abnormality and ocular misalignment.Surgery for exotropia resulted in cosmetic improvement but no stereopsis. There is a possibility that better outcome would have been achieved if the diagnosis and treatment could be made during infancy. We advocate early detection of periventricular leukomalacia and treatment of eye complications.
