糖尿病例において,小切開創白内障手術終了時のステロイド結膜下注射(以下,結注)の術後炎症軽減効果を検討した。小切開創白内障手術を受けた糖尿病例86例86眼を対象とし(増殖糖尿病網膜症と術中合併症例は除外),ステロイド結注施行群52眼と非施行群34眼とに分けた。前房フレア値を術前,術翌日,2日,5日,7日,14日後に測定し,2群間で比較したが,いずれの検査時期においても2群間で差がなかった。したがって,糖尿病例においても,増殖糖尿病網膜症や術中合併症がない場合,白内障手術終了時のステロイド結注は不要と考えられた。
We evaluated the effect of subconjunctival corticosteroid injection immediately after cataract surgery in diabetic eyes. A series of 84 diabetic patients underwent routine small-incision cataract surgery.They were randomized into two groups. One group of 52 eyes received subconjunctival dexamethasone and gentamicin and the othr 34 eyes received gentamicin only. Cases of proliferative diabetic retinopathy and those with intraoperative complications were excluded.The aqueous flare value was evaluated using a laser flare meter before surgery and 1, 2, 5, 7, and 14 days after surgery. There were no differences in the aqueous flare values between the two groups throughout.The findings show that subconjunctival corticosteroid has no beneficial value in reducing postoperative intraocular inflammation after cataract surgery in diabetic eyes.
