Pupillary dynamics was evaluated by an iriscorder after cataract surgery using flexible iris retractor in 12 eyes with miotic pupil. The findings were compared with 20 eyes which underwent cataract surgery without iris retractor. The eyes in the former group showed significant increase in minimum pupil area after light stimulus and retarded responses regarding contraction ratio, maximum velocity of contraction and dilation. The initial pupil area was significantly increased in eyes with longterm topical pilocarpine and in eyes with pupil area of more than 25 mm2, suggesting less movable and atonic pupil. Decrease in contraction ratio persisted long after surgery, suggesting an irreversible dysfunction of pupillary sphincter muscles. Above findings show that the use of iris retractor for miotic pupil during cataract surgery may induce tear of pupillary sphincter with pupillary dysfunction. Excessive stretch of sphincter muscles should be avoided particularly in eyes with extreme miosis or organized pupil margin.
