先天緑内障眼に生じた巨大裂孔網膜剥離の1例

Giant retinal break in a case of congenital glaucoma

　13歳の男児が，右眼の著明な増殖硝子体網膜症を伴う巨大裂孔を伴った網膜全剥離のため，当科に紹介された。生後約1か月に，先天緑内障のため両眼の線維柱帯切開術を受けていた。硝子体手術，シリコンオイルタンポナーデを施行したが，術後8日目に再増殖による再剥離が出現したため，再度，硝子体手術，シリコンオイルタンポナーデを施行し，網膜は復位した。右眼眼軸長は33.8mmと延長しており，術中に硝子体が液化している所見もみられ，これらが巨大裂孔を伴った増殖硝子体網膜症の誘因と考えられた。先天緑内障眼において網膜剥離の合併した報告例は少数ではあるが，視力予後不良であり，定期的な眼底検査が必要であると考えられた。

A 13-year-old boy was referred to us for total retinal detachment with a giant retinal break and advanced proliferative vitreoretinopathy in his right eye. He has undergone bilateral trabeculotomy for congenital glaucoma at one month after birth. The affected eye was treated by vitrectomy and silicone oil tamponade.The retina became eventually reattached after a similar procedure performed one week later for recurrence of retinal detachment with vitreoretinal proliferation.The axial length of the affected eye was 33.8mm with remarkably liquefied vitreous.These features appeared to have induced giant retinal break and proliferative vitreoretinopathy.
