We performed vitreous surgery for macular hemorrhage secondary to retinal arterial macroaneurysm in 11 eyes of 11 patients. The hemorrhage was located anterior to the retina in 4 eyes and in the subretinal space in 7 eyes. Surgery was performed 7 to 30 days after onset, average 20 days. Tissue plasminogen and intravitreal perfluorocarbon were also used in 5 eyes with submacular hemorrhage. The visual acuity improved in 9 of the 11 eyes after surgery. Visual acuaity of 0.5 or over was achieved in 4 eyes. The final visual acuity was less than 0.1 in 3 of 5 eyes after removal of submacular hemorrhage. Final visual acuity of 0.5 was attained in only one out of 6 eyes in patients aged 80 years or over. The findings show that vitreous surgery may not induce improved visual acuity when the patient is aged 80 years or over.
