特発性黄斑上膜に対する硝子体手術後の中心窩網膜厚と網膜感度

Foveal thickness and retinal sensitivity following surgery for idiopathic epiretinal membrane

　特発性黄斑上膜15眼に対する硝子体手術を行った。視力，中心窩網膜厚，中心窩網膜感度を術前，術後1,2,3,6か月に測定した。中心窩網膜厚はRetina Map^TM，中心窩網膜感度はハンフリー視野計を用いて測定した。術後，視力は有意に改善し，網膜厚は有意に薄くなったが，網膜感度は改善しなかった。術前は中心窩網膜厚および網膜感度と視力は有意に負の相関を示した。術後は相関しなかったが，術後後期ほど負の相関傾向を示した。内境界膜剥離眼は非剥離眼と比較して，視力改善度には差がなかったが、中心窩網膜感度の改善度は有意に悪かった。

Vitrectomy with membrane peeling was performed in 15 eyes of idiopathic epiretinal membrane. Each case was evaluated for visual acuity, foveal thickness and central retinal sensitivity before surgery and 1, 2, 3 and 6 months after surgery. Foveal thickness was measured by a retinal thickness analyzer (Retina Map^TM) Central retinal sensitivity was measured by Humphrey Visual Field Analyzer 630. Following surgery, visual acuity improved and foveal thickness decreased significantly (p ＜ 0.05) but the central retinal sensitivity did not improve. The foveal thickness and central retinal sensitivity showed no correlation with the visual acuity during the immediate postoperative period and showed negative correlation later. Improvement in central retinal sensitivity was significantly better in 10 eyes which the internal limiting membrane (ILM) was left intact than in 5 eyes which underwent removal of ILM. There was no difference in improvement of visual acuity between the two groups.
