成人と小児における増殖硝子体網膜症の手術成績

Outcome of surgery for proliferative vitreoretinopathy in children and adults

要約　目的：小児と成人の増殖硝子体網膜症（PVR）に対する手術成績の比較。対象と方法：過去4年間に硝子体手術を行った増殖硝子体網膜症25例27眼を解析した。8眼が15歳未満の小児，19眼が15～85歳の成人であった。結果：網膜の復位は小児では7眼中4眼（57％），成人では18眼中17眼（94％）で得られた。復位率は2群間で有意差があった（p＜0.05）。0.1以上の最終視力は，小児では2眼（25％），成人では13眼（72％）で得られた。この値には2群間に有意差があった（p＜0.05）。結論：15歳未満の小児での増殖硝子体網膜症は，これ以上の年齢層と比べ，手術による復位が困難で，最終視力が不良である。

Abstract.　Purpose：To report the outcome of surgery for proliferative vitreoretinopathy（PVR）in children and adults. Cases and Method：This study was made on 27 eyes of 25 patients who received vitreous surgery for PVR during the past 4 years. The series comprised 8 eyes of children under 15 years of age and 19 eyes in adults aged between 15 and 85 years. Results：The retina became reattached in 4 out of 7 eyes（57％）in children and in 17 out of 18 eyes（94％）in adults. The difference was significant（p＜0.05）. Final visual acuity was 0.1 or better in 2 eyes（25％）in children and in 13 eyes（72％）in adults. The difference was significant（p＜0.05）. Conclusion：Final visual acuity after surgery for PVR was poorer in children under 15 years of age than in adults aged 15 years or over.
