光学的干渉断層計による網膜前黄斑線維症の観察

Optical coherence tomographic features of preretinal macular fibrosis

　網膜前黄斑線維症の網膜断面を光学的干渉断層計OCTで観察した。生理的な中心窩陥凹は，網膜厚の増加により消失していた。偽円孔例では，中心窩の周囲の網膜が厚くなっているため生理的陥凹が修飾され，円筒状に強調されていた。これら症例では，観察光の反射が網膜外層で低下しており，浮腫あるいは膨化があると考えられた。本症では網膜表面の膜形成のみならず，膜による網膜内の構造変化が合併していることがわかった。本症の病態の解釈にOCTは必須である。

Optical coherence tomography (OCT) was performed on 19 eyes of idiopathic preretinal macular fibrosis. There was an increase of foveal thickness which ranged from 300 to 650 gm. The swollen macula showed anterior protrusion and lacked the physiological foveal depression. In eyes showing pseudomacular hole, the foveal surface showed columnar depression surrounded by thickened perifoveal retina. Swelling of the retina was more pronounced in the outer retina which showed low reflectivity. The findings seemed to show that preretinal macular fibrosis does not merely involve the retinal surface but is also associated by fluid accumulation in the outer retinal layers.
