黄斑浮腫の光学的干渉断層計所見

The macular edema evaluated by optical coherence tomography

　黄斑浮腫の網膜断面構造を，光学的干渉断層計（Optical coherence tomography：OCT）で検索した。対象は黄斑浮腫がある56眼，44例（男子27例，女子17例）である。56眼の内訳は，糖尿病網膜症43眼，網膜静脈分枝閉塞症6眼，網膜中心静脈閉塞症3眼，ぶどう膜炎2眼，後部強膜炎1眼，粟粒血管腫症1眼である。OCTの所見から，黄斑浮腫は，網膜の膨化50眼，嚢胞様黄斑浮腫（cystoid macular edema：CME）28眼，漿液性網膜剥離4眼の3つの型に分類された。これらの型は，しばしば同一眼で合併していた。網膜の膨化は主として網膜外層にあり，膨化が強いとスポンジ状になっていた。新鮮なCME例では網膜外層の嚢胞によって中心窩が突出し，網膜内層の層構造は比較的保たれていた。陳旧化したCMEでは，嚢胞が網膜の外層から内層を占めており，黄斑部は嚢胞によって空洞化していた。硬性白斑は高反射領域として写り，網膜の外層だけでなく，内層にもあった。CMEではない黄斑浮腫の中心窩の網膜厚と視力は，中等度の負の相関を示した。

We evaluated the cross-sectional image of various types of macular edema in 56 eyes using an optical coherence tomography, OCT. The series comprised diabetic retinopathy 43 eyes, branch retinal vein occlusion 6 eyes, central retinal vein occlusion 3 eyes, uveitis 2 eyes, posterior scleritis 1 eye and miliary angiomatosis 1 eye. The OCT findings showed three patterns of macular edema : sponge-like swelling of the retina (50 eyes), cystoid macular edema (28 eyes) and serous retinal detachment (4 eyes) . More than one pattern was often present in the same eye. Retinal swelling was more pronounced in the outer than the inner layer of the retina. Cystoid macular edema was mainly located in the outer retinal layer and, in persistent cases, cysts were fused together resulting in a large cystic cavity involving the entire retinal layers. Hard exudates appeared as high-reflective areas located in the outer and inner retinal layers. In eyes without cystoid macular edema, there was a medium negative correlation between the foveal thickness and visual acuity.
