腎移植後に発症したサイトメガロウイルス網膜炎

A case of cytomegalovirus retinitis following kidney transplantation

（17-P1-8）　53歳の女性が，約4年前に水腎症による慢性腎不全のために腎移植を受けた。慢性拒絶反応のために4か月前に2度目の腎移植を受けた。慢性拒絶反応のためにシクロスポリンの全身投与を受け，3週間前からタクロリムスを内服している。1週間前からの飛蚊症で当科を受診した。両眼のサイトメガロウイルス網膜炎と診断された。ガンシクロビルの内服で眼底病変は消退しつつあったが，左眼の萎縮網膜に円孔が生じ，網膜剥離が発症した。硝子体手術とシリコン充填で網膜は復位した。以後ガンシクロビル内服で眼底病変は沈静化し、タクロリムスの減量が可能になった。ガンシクロビルの中止以後，網膜炎は再燃していない。

A 53-year-old woman had received kidney transplantation for chronic renal failure 4 years before. Chronic rejection necessitated a second kidney transplantation 4 months before. She was thereafter treated by systemic cyclosporin which was switched to tacrolimus 3 weeks before. Floaters led to the detection of cytomegalovirus retinitis in both eyes. Systemic ganciclovir induced regression of retinitis. Ensuing rhegmatogenous retinal detachment was successfully treated by vitreous surgery with silicone oil tamponade. There has been no recurrence of retinitis after withdrawal of ganciclovir.
