両側視神経乳頭腫脹をきたしたサルコイドーシスの1例

A case of sarcoidosis with bilateral optic disc swelling

（18-G409-25）　サルコイドーシスによる両側視神経乳頭腫脹をきたした1例を経験した。症例は22歳男性で，ステロイド40mgからの漸減投与中に前部ぶどう膜炎と血管炎は軽快したが，両視神経乳頭腫脹は悪化した。乳頭腫脹の悪化に対してプレドニゾロンを100mgに増量して漸減投与したが，乳頭腫脹の反応は不良であった。その後ステロイド40mgを約5か月，プレドニゾロン換算6,415mg投与で乳頭腫脹は軽減した。本症例の乳頭腫脹悪化には不十分なステロイド投与と，前部ぶどう膜炎と乳頭腫脹のステロイドの反応性の違いが関与している可能性があり，乳頭病変を伴ったサルコイドーシスにはステロイドの大量かつ長期漸減投与が必要と考えられた。

We treated a 22-year-old male with ocular sarcoidosis. Systemic treatment with daily dosis of prednisolone resulted in improvement of anterior uveitis and retinal vasculitis but aggravation of bilateral optic disc swelling. The swelling failed to respond after increase of daily dosis of prednisolone to 100 mg. It finally improved after continued administration of daily dosis of prednisolone totalling 6,415 mg. This case shows the possibility that anterior uveitis and optic disc swelling in ocular sarcoidosis have difference in responsiveness to systemic corticosteroid, the latter necessitating a higher and longer administration.
