インターフェロン治療中に発症した外転神経麻痺の1例

Abducens nerve palsy in a case under treatment with systemic interferon

　インターフェロン投与中に片眼の外転障害を起こした1例を経験した。症例は42歳男性で，C型慢性肝炎に対しインターフェロン（IFN-α）投与開始後17日目に，同側性複視を伴う眼球運動障害を生じ，右外転神経麻痺と診断された。インターフェロン投与中止後，外転障害は急速に軽減し，1か月で複視は完全に消失した。MRI画像診断を含む神経学的検索によっても外転制限の原因は特定できず，外転神経麻痺はインターフェロン治療による副作用であることが示唆された。インターフェロン治療患者の診療において，網膜症とともに，眼球運動神経麻痺にも注意を払う必要があると考えた。

A 42-year-old male developed right abducens nerve palsy. He had been under treatment with systemic interferon- a for chronic hepatitis C since 17 days before. The abducens palsy rapidly improved after discontinu-ation of interferon. Diplopia disappeared one month after discontinuation. Neurological examinations including magnetic resonance imaging failed to identify the cause of abducens palsy. This case illustrates that oculomotor narve palsy is a liability in patients under treatment with systemic interferon.
