Abstract Purpose: To report a case of syphilitic uveitis who presented bilateral altitudinal hemianopsia.

Case: A 66-year-old man presented with impaired vision in both eyes since 4 months before. He had reportedly visual acuity of 0.7 in either eye before onset of visual impairment.

Findings and Clinical Course: Best-corrected visual acuity was 0.2 right and 0.6 left. Both eyes showed reduced light reaction, vitreous opacity, choroidal atrophy, and white dots around the disc. Goldmann perimetry showed superior altitudinal hemianopsia in both eyes. He was diagnosed with neurosyphilis based on positive serous test, elevated monocytes and protein in the cerebrospinal fluid, and MRI findings of the cerebrum. Infusion of corticosteroid and massive dosis of penicillin was by improved vision and subjective findings the next day, and by improved visual field 13 days later. Visual acuity is maintained at 0.7 right and 1.0 left for 21 months until present.

Conclusion: This case illustrates that visual field may be affected in syphilitic uveitis probably due to underlying neurosyphilis.
