A 77-year-old male had undergone extracapsular cataract extraction in the left eye 5 years before. Secondary intraocular lens implantation was made 3 years later. Another 6 months later, astigmatic keratotomy was performed. He presented with blurring of vision 2 weeks after astigmatic keratotomy. He had been under treatment for diabetes with poor control during the last 10 years. No diabetic retinopathy was present . He now showed corneal ulcer with hypopyon. Treatment with systemic and topical antibiotics resulted in improvement of the corneal lesion 2 weeks later. Methicillin-resistant Staphylococcus aureus (MRSA) was isolated from the corneal scrapings. This case illustrates the possibility of corneal infection following astigmatic keratotomy.
