乱視矯正角膜切開術後に角膜潰瘍を発症した1例

A case of corneal ulcer after astigmatic keratotomy

（25D-14）　現在77歳の男性が5年前に左眼の嚢外白内障摘出術を受けた。その3年後に眼内レンズの二次挿入術が行われ，さらに6か月後に乱視矯正角膜切開術が行われた。その2週後に視力障害を自覚して受診した。患者は10年前から糖尿病の加療中であり，コントロール不良であった。糖尿病網膜症はなかった。診察時には角膜潰瘍があり，前房蓄膿を伴っていた。抗生物資の全身と局所投与により，2週後に角膜所見は改善した。経過中．潰瘍部の擦過培養でメチシリン耐性黄色ブドウ球菌が検出され，これによる細菌性角膜潰瘍と診断された。乱視矯正角膜切開術により術後の角膜感染が起こりうることを示す症例として注目される。

A 77-year-old male had undergone extracapsular cataract extraction in the left eye 5 years before. Secondary intraocular lens implantation was made 3 years later. Another 6 months later, astigmatic keratotomy was performed. He presented with blurring of vision 2 weeks after astigmatic keratotomy. He had been under treatment for diabetes with poor control during the last 10 years. No diabetic retinopathy was present . He now showed corneal ulcer with hypopyon. Treatment with systemic and topical antibiotics resulted in improvement of the corneal lesion 2 weeks later. Methicillin-resistant Staphylococcus aureus (MRSA) was isolated from the corneal scrapings. This case illustrates the possibility of corneal infection following astigmatic keratotomy.
