多胎児における未熟児網膜症

Retinopathy of prematuriy in multiple-birth infants

（25A-3）　1991年7月〜1996年8月に宮崎医科大学附属病院眼科を受診した多胎新生児35組77例（双胎28組，品胎7組）における未熟児網膜症について報告した。未熟児網膜症とそれに影響を与えると予想される諸因子をレトロスペクティブに調査し，特に同胞間で未熟児網膜症のステージに差のある症例を対象に増悪因子について検討した。出生体重，アプガースコア1分値，サーファクタント投与が未熟児網膜症の発症と相関しており，特にアプガースコア1分値は増悪とも相関していた。出生時の全身状態が，未熟児網膜症の発症および増悪に大きく関与していると考えられた。

We reviewed 77 cases of 35 pairs of multiple birth infants seen by us during the past 5 years and 2 months. The series comprised 28 pairs of twins and 7 sets of triplets. We evaluated the birthweight, Apgar score, supplemental oxygen, infantile respiratoty distress syndrome and the grade of retinopathy of prematurity, ROP. In 28 cases of 12 pairs of twins or triples, differences were present in the severity of ROP betweeen or among the siblings. Birthweight, Apgar score and infantile respiratory distress syndrome were significant factors in the occurrence of ROP in these cases. Apgar score was also significantly involved in the progression of ROP.
