網膜神経線維層欠損と視神経乳頭の形態の関係

Correlation between the shape of optic nerve head and retinal nerve fiber layer defect

要約　目的：正常眼圧緑内障での網膜神経線維層欠損と視神経乳頭の形態との関連の検索。対象と方法：正常眼圧緑内障17眼を対象とした。男性7例11眼，女性4例6眼で，年齢は39～73歳（平均57歳）であった。走査レーザー検眼鏡（SLO）で全例に網膜神経線維層欠損があった。視神経乳頭の形態はハイデルベルク網膜断層計で解析し，神経線維層欠損に接する部位と接しない部位について，rim area，rim volume，cup/disk area ratioをそれぞれ算出した。結果：網膜神経線維層欠損に隣接する視神経乳頭の部位が，隣接しない部位よりも緑内障性変化が強い傾向があったが有意差はなかった。結論：正常眼圧緑内障では，網膜神経線維層の構造的な変化が視神経乳頭の形態の変化に先行する可能性がある。

Abstract.　Purpose：To study the correlation between the shape of optic nerve head and retinal nerve fiber layer defect（NFLD）in eyes with normal-tension glaucoma. Cases and Method：This study was made on 17 eyes with normal-tension glaucoma. The series comprised 11 eyes of 7 males and 6 eyes of 4 females. Their ages ranged from 39 to 73 years,average 57 years. All the eyes had NFLD by scanning laser ophthalmoscope. Shape of optic nerve head was analyzed by Heidelberg retina tomograph to calculate rim area,rim volume,and cup/disk area ratio as parameters. Results：There was no significant difference in the shape of optic nerve head between areas adjacent to NFLD and other areas. Conclusion：There is a possibility that morphological changes of retinal nerve fiber layer may precede those of optic nerve head in normal-tension glaucoma.
