著明な視神経乳頭浮腫を伴った小児ぶどう膜炎の1例

Severe optic disc edema in a child with uveitis

　急激な視力低下を主訴とし，著明な視神経乳頭浮腫を伴った片眼性の小児ぶどう膜炎を経験した。10歳女児で左眼視力低下，発熱を主訴に来院した。視力は右1.2（n．c．），左0.02（n．c．），眼圧は左右とも16mmHg，右眼には異常なく，左眼には前房および硝子体内に炎症細胞が浮遊し，眼底には乳頭上方の強い滲出性変化と，肉芽腫様病変を伴った特徴あるぶどう膜炎であった。視神経炎症状を呈しているため，ステロイド大量療法を開始したところ，乳頭上方の肉芽腫様腫瘤が明瞭となった。全身検査所見では特に異常なく，Shieldsの分類によるトキソカラ症のoptic papillitis型と推定した。ジエチルカルバマジンを併用したステロイド大量療法にて左視力1.0まで回復し，肉芽腫様病変は退縮した。本症例は著明な視神経乳頭浮腫を伴った肉芽腫性病変を有するぶどう膜炎で，今回の治療法は極めて有効であった。

A 10-year-old girl presented with fever and blurred vision in the left eye. The visual acuity was1.2 right and 0.02 left. The right eye was intact. The left eye showed inflammatory cells in the aqueous andthe vitreous. Severe exudative lesion was present superior to the swollen optic disc. The condition wasdieagnosed as granulomatous uveitis. Systemic treatment with massive corticosteroid was followed bymanifestation of granulomatous tumor superior to the disc. Laboratory tests were equivocal. The conditionwas diagnosed as optic optillitis due to toxocara after Shields. Systemic corticosteroid with adjunctivediethylcarbamazine resulted in resolution of granulomatous lesions and in improvement of left visual acuityto 1.0.
