僚眼に視野異常を認めた桐沢型ぶどう膜炎の1例

Unilateral acute retinal necrosis with visual field defect in the fellow eye

　片眼性の桐沢型ぶどう膜炎の僚眼の視機能に異常を認めた報告は筆者らが検索した限り，いまだない。今回筆者らは，片眼性の桐沢型ぶどう膜炎患者の検眼鏡的に異常のない僚眼に，進行性の視野異常を認めたので報告する。

　症例は，34歳の男性で1990年1月4日に右眼の急激な視力低下をきたし，当科を初診した。アシクロビルの投与を開始するも1月26日には右眼に網膜剥離を生じた。2月2日の左眼の静的量的視野検査にて異常を認め，その後，耳側視野異常の進行を認めた。5月29日からアシクロビルの内服を開始したが，その後，左眼視野異常の改善を認めた。

　片眼性の桐沢型ぶどう膜炎の僚眼に対しての視機能評価の必要性が示唆された。

A 34-year-old male sought medical advice due to acute visual loss in his right eye. He manifested typical clinical features of acute retinal necrosis syndrome and wan treated by systemic acyclovir. Retinal detachment developed on day 20 of the disease. Surgery with encircling scleral buckling was futile. Visual field defect was detected in the apparently healthy left exe on day 27. The field defect progressed to involve the temporal periph-ery. Systemic acyclovir therapy was reinstituted 15 weeks later. The visual field improved to normal level 3 months later. The finding seemed to imply that visual field defect may be the first clinical sign of acute retinal necrosis even in the absence of detectable funduscopic lesions.
