視神経乳頭drusenによる乳頭浮腫の1例

A case of optic disc drusen simulating longstanding papilloedema

緒　言

　一般に視神経乳頭drusenは，検眼鏡で，乳頭部に黄白色に輝く塊状物として見られるが，乳頭深層に発生すると，検眼鏡でそのものは見えず，乳頭浮腫を示すことがあるとされている^1〜3）。われわれは4年間片眼に乳頭浮腫が持続し，その間眼底所見，視力，視野所見が，初診時と同じ状態が続いている症例を経験し，種々の検査ことに螢光眼底検査と超音波検査によつて，視神経乳頭深部に形成されたdrusenによる乳頭浮腫と診断した。

　視神経乳頭drusenは，わが国では報告の少ない疾患であるが，今後視神経疾患の一つとして，ことに乳頭浮腫の原因として，考慮されねばならないと思われるので，この症例を紹介する。

A 38-year-old female manifested unilateral papilloedema accompanied by transient amauro-sis and a visual field defect in the lower nasal periphery. The visual acuity was normal. The fundus appearance and visual functions remained unchanged during the following 4 years. Routine neurological examinations revealed no abnorma-lity.

Fluorescein angiography showed hyperfluore-scence in the disc margin in the late venous phase. The prepapillar capillaries were not engorg-ed and did not manifest dye leakage. B-scan ultrasonography showed a prominent echo and internal reflections in the optic nerve head.
