We developed a new trabeculectomy method intended to assure safety and to prevent postopera-tive ocular hypotony. A scleral flap is made after the traditional method. A blade is inserted into the anterior chamber through the corneoscleral margin. The iris base is widely excised. A newly devised trabeculectomy -punch is inserted between the sclera and the uvea. The scleral fragment including the trabecula is punched out together. After resetting the iris, the center of the posterior surface of the inverted scleral flap is cauterized horizontally using a Wetfield cauterizing terminal (Tane's method). No fibroblast growth inhibitor is used. The whole procedure is performed under an operating microscope. A total of 13 eyes with glaucoma were operated by this method. The pressure-lowering effect was satisfactory. Postoperative side effects, including ocular hypotony, were markedly less. No significant differences were noted in histopath-ological findings in 30 rabbit eyes operated by this method with or without mitomycin C.
