難治性黄斑円孔に対する硝子体再手術と自己血清滴下

Use of autoserum during vitreous resurgery for persistent macular hole

　難治性黄斑円孔に対し，自己血清を滴下することで良好な成績を得た。対象は初回硝子体手術を行った25眼中，液—気体置換術を追加しても治癒しなかった難治例で，再手術の同意が得られた5眼である。術直前に採血しておいた自己血清を0.1〜0.3ml黄斑部に滴下し，平均7分間留置した。再手術を行った5眼中4眼で解剖学的な円孔の閉鎖が得られ，5眼中3眼では2段階以上の視力の改善がみられた。従来の黄斑円孔手術における液—気体置換に自己血清の滴下を追加することにより，難治性黄斑円孔においても円孔閉鎖が期待できることが明らかとなった。

We treated 25 eyes with macular hole by vitreous surgery with air tamponade. Macular hole failed to heal in 9 eyes. Five out of these 9 eyes were treated by another vitreous surgery by placing autoserum for 7 minutes. Anatomical cure was obtained in 4 eyes. Visual acuity improved by 2 lines or more in 3 eyes.Use of autoserum was thus effective for macular holes which failed to respond to conventional vitreous surgery with air tamponade.
