特発性黄斑円孔の術後経過

Outcome of surgery for idiopathic macular hole

　硝子体手術を行った特発性黄斑円孔35眼について，円孔閉鎖率，術後視力，合併症を検討した。12か月以上経過観察できた17眼の平均経過観察期間は19か月で，最長30か月であった。初回手術での閉鎖率は83％，最終的な円孔閉鎖率は89％であった。円孔の閉鎖した31眼では退院時にすべて術前以上の視力となり，その後も視力は徐々に回復した。35眼全例で，術後視力が術前より2段階以上低下した症例はなかった。術後，35眼中21眼（60％）に網膜前膜が形成された。それらの膜は菲薄なものが多く，視力に影響を与えることはなかった。

We reviewed 35 eyes of idiopathic macular hole following vitrectomy followed up for 12 to 30 months, average 19 months. The macular hole was closed in 29 eyes (83 %) after one surgical session and in 31 eyes (89%) after one or more sessions. The visual acuity gradually improved in the 31 eyes with closure of the hole. None of the 35 eyes showed decreased visual acuity after surgery. Epiretinal membrane formed in 21 of the 35 eyes (60%) after surgery but did not interfere with the course of visual recovery.
